1. Terms and Conditions: The Applicant for “Licensed Ecclesiastical Holistic Practitioner” (“Practitioner” or “Licensee”) requesting membership to receive an ecclesiastical ministerial (License) be issued to practice holistic wellness (Ecclesiastical Wellness) be provided to her/him by the Guardian Ecclesiastical Holistic Association™, which is a d/b/a of Guardian Ecclesiastical Holistic Association, Inc., (hereafter referred to as “GEHA” or “Association”) a not-for-profit IRS approved 501 c-3 religious, educational, charitable and scientific corporation, domiciled in Shreveport, Louisiana, USA (333 Texas St, Suite 1300, Shreveport, LA 71101) do hereby agree to the terms and conditions of this agreement as follows:
2. Code of Operations: The Practitioner hereby agrees to adhere and abide by the “Code of Operations,” as set forth by GEHA which acknowledges he/she has fully read and understands.
3. Eight Essential Biblical Truths: The Practitioner agrees to adhere to GEHA’s “Eight Essential Biblical Truths,” as set forth by GEHA which acknowledges he/she has fully read and understands.
4. LifeCare Agreement: (Practitioner/Member). The Practitioner agrees to adhere to the “LifeCare Agreement,” as set forth by GEHA which acknowledges he/she has fully read and understands.
5. Time Limits to Cancel Agreement: It is agreed to by both parties that the “Practitioner” has the right to cancel this Agreement after a “three day waiting period from the date the admissions fee has been paid. Otherwise, if not cancelled during the “three day waiting period,” this Agreement shall remain in effect.
6. Directory Listing: Holistic Health Link (HHL): “Practitioner” agrees to pay Holistic Health Link (located in the state of Florida), a monthly fee of $25.00 as agreed in addition to his/her monthly GEHA membership fee. GEHA contracted with Holistic Health Link (HHL) to provide a location for posting the LifeCare Agreement. On each individual HHL profile, there is a tab to access the digital form and a tab for the printable version. In addition, on each HHL dashboard, a digital database collects the contact information of all forms submitted on the profile. HHL ONLY stores the contact information, no personal or medical information.
Note: Holistic Health Link is a third-party company that has no obligation to monitor how you choose to use its platform and is not responsible for any unauthorized access or loss of information and shall not be liable for any loss or damage which may occur as a result of any virus or data breach of security.
7. Membership Agreement/Subscription Cancellation: Once you are approved for a GEHA License and have paid the appropriate fees, you may cancel your GEHA Subscription (License membership) at any time directly through your GEHA dashboard. If you cancel (or your account is cancelled due to failed payment), your GEHA License immediately becomes null and void; however, your account details on the GEHA website shall remain for reactivation purposes. Your HHL – Holistic Health Link Listing (directory listing) will be auto-migrated to a “non-GEHA” listing (at the same cost) and all GEHA elements will be removed (GEHA logo, disclaimer, LifeCare Agreement tabs, GEHA Database on dashboard). You can cancel your HHL subscription anytime on your HHL dashboard.
8. Suspended Membership Subscription: In the event that your GEHA Membership Subscription payment is due but your credit card on file is no longer valid or active, you should update your credit card information in your account and/or we may attempt to contact you to determine updated account information. We will wait ten (10) days before canceling your membership due to failed payment. You can reactivate your membership at any time (see below).
9. Subscription and Membership Reactivation: If you have an active HHL (Holistic Health Link Directory listing) Subscription, upon reactivation of your GEHA Membership from your GEHA account, your GEHA License shall become immediately active. If you do not have an active HHL Subscription, you must reactivate both your HHL Subscription and GEHA Membership from your GEHA account. Once both your HHL Subscription and GEHA Membership are reactivated, your GEHA Credential (Licensed Practitioner) shall immediately become active. If you reactivate your GEHA Credential within your current billing cycle (that is, when your payment due date has not passed), your existing billing cycle will remain. If you reactivate your GEHA Credential outside your billing cycle (that is, when your payment due date has already passed), you will be redirected to the cart to reactive (re-purchase) your Credential to start a new billing cycle. Your Holistic Health Link listing will auto-migrate back to a GEHA listing and all GEHA elements will be added (GEHA logo, disclaimer, LifeCare Agreement tabs, GEHA Database on dashboard).
10. GEHA’s Logo and Website: As a Licensee, the GEHA logo is automatically posted at the top of your Holistic Health Link profile. You may also use it on your business cards without permission – jpg and png files are posted at the top of the “member’s only” page, which can be accessed by logging in to your GEHA dashboard. Any other use of the logo requires permission by sending a request to info@gehassociation.org. This Agreement oversees your right for the use of the GEHA website for content, public and protected login areas.
11. Annual Report Renewal: Each year, on the 365-day anniversary of your GEHA Membership, as a Licensee you will be asked to submit an annual report to renew your Credential. You will receive an email with respective link to complete the annual report form. This form must be completed and submitted within 30 days of receipt. You will be asked to upload your most recent liability insurance declaration page that illustrates proof of the required insurance. If you do not submit your form within 30 days of receipt, your GEHA Credential (License) shall be suspended until such annual report renewal and proof of insurance are provided to GEHA. The monthly GEHA fee could increase or decrease based on new information on the annual report.
New applicants who apply and are denied a Credential (License) shall have the right to appeal to the President of GEHA. The appeal must include the reason and all grounds for the appeal. The appeal will be considered with all fairness to the applicant and/or current member in accordance to GEHA’s Polices, Licensed Agreement and Code of Operations. If, after careful consideration, the applicant’s Credential is still denied, this decision will be final and a second appeal is not allowed for the applicant or member to renew.
12. Requirement to keep Possession of the GEHA Lifecare Agreements: You agree to keep possession in digital or physical files of all signed Lifecare Agreements contracts for a period of five (5) years but no less than three (3) years from the date of the last holistic service you provided your clients (non-credentialed GEHA “Members”). GEHA contracted with Holistic Health Link to provide a digital database to keep (only) the contact information of all member clients who fill out the form online.
The Fourteenth Amendment of the US Constitution prohibits any states from depriving any person of life, liberty, or property without due process of law, and guarantees all persons equal protection of (that is, by or under) the laws. “We, as members (client members) of GEHA declare the right to assemble (or congregate) as a community to choose our options wellness care and how our personal information and records are preserved and protected, and to decide what wellness options are best for us.” (NOTE: This statement does not remove the practitioner from the requirement to be state-licensed, if a state license is required for the services the practitioner provides.) Further, if a state licensing board has the authority to issue a subpoena, as many of them do, a private agreement regarding records will not preempt a state agency from obtaining records through a subpoena. Additionally, if a court views these records as clergy records, the client likely has the right to waive any privilege (depending on the state) and allow the records to be used for legal proceedings. The client always has the right to a copy of his/her records and to release to whomever his/her desire, or he/she can sign a release/waiver form for the practitioner to release his/her records to whomever the client authorizes.
13. Indemnification: The Licensee shall indemnify and hold harmless, and hereby does indemnify and hold harmless GEHA and its respective affiliates, directors, officers, advisory board, employees, agents and representatives from and against any and all claims, demands, suits, causes of action, damages, losses, cost and expenses, including reasonable attorney fees and fines, penalties that arise directly or indirectly from your actions or omissions in carrying out this agreement including but not limited to your service as a Licensed Ecclesiastical Holistic Practitioner. You also agree to hold harmless GEHA for any actions of your employees that act under your supervision in providing services to clients.
14. Required Insurance: While Licensees may gain certain protections under the terms of the GEHA Lifecare Agreement; GEHA does not cover or provide insurance to Licensees. GEHA requires all Licensees in the U.S. to have a professional liability insurance policy for any services the Licensees offer inside or outside the GEHA Code of Operation. Such policy must list GEHA as an additional insured “Guardian Ecclesiastical Holistic Association, 333 Texas St, Suite 1300, Shreveport, LA 71101.”
You hereby agree to obtain and maintain such coverage at all times while your GEHA License is in effect. If your current insurance covers the Ecclesiastical Holistic Services that the GEHA License allows, you do not need to purchase an additional policy. However, you are required to add GEHA as an additional insured. Proof of insurance (declaration page) must be submitted within 30 days of paying your GEHA fees to insurance@gehassociation.org. You will be asked to resubmit an updated declaration page when your annual report is due (every 365 days). Failure to maintain insurance shall subject the Licensee to a license suspension until proof of insurance is provided. Failure to maintain adequate insurance to cover “all” holistic services you provide shall be cause for license termination.

15. Boundary of License Authority: You are commissioned and ordained as a Licensed Ecclesiastical Holistic Practitioner and the Ecclesiastical Ministerial License granted by GEHA to you authorizes you to operate with the boundary of your educational training as a holistic wellness practitioner. At all times, you are required to operate in accordance to your religious rights provisions in the US Constitution and abide by the laws of your state. You are encouraged to consult, give lectures, seminars, teach as special events, as well as endeavor to bring people to God and live a holy life according to the Bible. You are to consult with your tax professional as to any tax benefit these ministerial credentials may or may not provide to you individually.
16. Disagreements and Discipline of Members: Both parties mentioned in this Agreement shall settle all disagreements concerning any matter (disputes, disagreements) within the confines of this Association (GEHA) under the governing laws and authority of the GEHA board. The “Practitioner” also agrees that if he/she becomes embroiled in a serious unsolvable disagreement with another “Practitioner,” both parties are required to contact the Board of GEHA at complaints@gehassociation.org. Both parties must endeavor to settle their differences in a Biblical manner within the Association, with the counsel of the Board of Directors of GEHA. The “Practitioner/Licensee” agrees to waive his/her right to file any kind of suit against GEHA through a Court of Law (or Judicial System). In addition, “Practitioner/Licensee” waives his/her right to file any kind of suit through a Court of Law against GEHA officers, employees, directors, advisory board, affiliates or representatives of GEHA.
17. Lifestyle and Discipline of GEHA Licensed Members – (General) GEHA is a Christian Association and therefore follows biblical truths and sets forth certain standards for membership. One who is a licensed member agrees to live a moral godly Christian life to the best of his/her ability according to the teachings in the Holy Bible. When the character, behavior, or doctrinal belief of a member becomes such as that not pertaining to what they originally agreed upon when his/her GEHA Ecclesiastical Ministerial License was issued and when counseling (from the GEHA Board) proves to be unproductive toward the reconciliation of differences, he/she will be required to surrender his/her ministerial credentials.
The goal of the GEHA Board concerning discipline is to render discipline in a loving, but firm, manner. Its objective is to correct and restore a member, and not to render punishment for any act committed. Disciplinary measures available to the GEHA Board may include, but are not limited to, the following:
(1) Verbal warning
(2) Written reprimand
(3) Probation, either with or without counseling
(4) Suspension from the Association for a fixed period of time; or
(5) Expulsion from the Association, with the option of revoking the guilty party’s ministerial credentials.
Upon investigation, any person found to be guilty of having materially misrepresented themselves in any of the application procedures for ministerial credentials shall be disciplined by the GEHA Board or dismissed from the GEHA membership.
18. Review of Agreement: “Practitioner” certifies and covenants that he/she has carefully read this Agreement throughout, and has had all of his/her questions thoroughly answered. Being of sound mind and having the wherewithal to understand the terms and conditions of the Agreement, I willingly enter into this agreement. You agree not to become a party with any other agreement that may conflict with this “Agreement Between Parties.”
19. Entire Agreement of the Parties: This Agreement (contract) supersedes any and all other agreements made, either oral or written, between the parties hereto with respect to services provided by the applicant as a credentialed “Practitioner” and contains all the covenants and agreements between the parties. Each party to this agreement acknowledges that no representations, inducements, promises, or agreements, orally or otherwise, have been made by any party, or anyone acting on behalf of any party and/or parties that are not embodied herein, and that no other agreement, statement, or promise not contained in this agreement shall be valid or binding.
The terms and conditions of this License agreement, Code of Operations and Lifecare Agreement may change from time to time by vote of the GEHA Board of Directors. Current practitioner credentialed members will be notified within 10 business days by email giving him/her the opportunity to review the updated revised version. If for any reason the licensee has questions about the updated revisions, please contact info@gehassociation.org.
20. Practitioner Member Email Address Use: Although GEHA will not sell or share your email address with a third-party, you agree to the following Use:
• Provide you with the Service(s).
• Send you communication from the Service(s).
• Send you requested product or service information
• Respond to user requests
• Administer your account
• Send you promotional and marketing communications
• Respond to your questions and concerns
• Facilitate your transactions with other users when you use our Service(s).
21. Acknowledgment: “Practitioner” acknowledges and warrants that he/she has read this Agreement in its entirety, and understands the aforementioned and agrees to abide by all the rules, codes, regulations, policies, terms, and conditions set forth in this Agreement.
